Abstract

Background: Chronic obstructive pulmonary disease (COPD) has a substantial impact on pulmonary function affecting the overall well-being of an individual. Barring pharmacological interventions; exercises play a crucial role in enhancing pulmonary function, exercise capacity, and health-related quality of life (QoL). This study aimed to estimate the effectiveness of exercise interventions in improving pulmonary function, exercise capacity, and quality of life in COPD patients.

Subject and methods: A meta-analysis was conducted using the PRISMA diagram and the research questions formulated in the PICO format. Population: COPD patients. Intervention: Exercises. Comparison: Standard care. Outcome: Pulmo­nary function (FEV1 and FEV/FVC scores), performance, and QoL. The articles extraction was from 2 databases, namely: PubMed, and ScienceDirect. The keywords used include "exercises" AND "pulmonary function" OR "exercise capacity" OR "quality of life" AND "COPD patients" AND “RCT”. The inclusion criteria in this study included full-text articles of randomized controlled trials (RCTs). Pulmonary function was measured by Forced Expiratory Volume 1 (FEV1) and FEV/FVC ratio. Performance was measured by 6–6-minute walking distance (6-MWD) Quality of life was measured using St. George's Respiratory Questionnaire (SGRQ). A higher score of SGRQ indicated a poorer quality of life. Data of pooled adjusted odds ratio (aOR) were calculated using the RevMan 5.4.1.

Results: The meta-analysis included 15 RCTs from Asia (12), Europe (1); North America (1), and South America (1). The total sample was 1,020 COPD patients. Exercises increased FEV1 (SMD= 0.25; 95% CI= 0.01 to 0.51; p= 0.050), FEV1/FVC (SMD= 0.31; 95% CI= -0.05 to 0.67; p= 0.090), performance (6MWD score) (SMD=0.58, CI95% (0.32 to 0.84); p=0.001), and quality of life (SGRQ score) (SMD= -0.60; 95% CI= -0.94 t0 -0.25; p=0.007).

Conclusion: Exercise of various modalities and durations holds a significant role as well as an integral component of COPD management; the intervention emerges as a promising pathway towards fostering healthier lives and improved well-being of COPD patients.
